Short-Form (21-item) Version

Upgrade Notice

Starting June 30th, 2021, the 54-item Fitness-to-Drive Screening Measure (FTDS) will no longer be available for use on the FTDS website. The tool will be replaced with the short form version of the tool called the "21-item Fitness-to-Drive Screening Measure Short-Form".

Research conducted by researchers at the University of Florida and University of Western Ontario using Google Analytics indicated the 20-minute completion time of the 54-item Fitness-to-Drive Screening Measure may be deterring users from completing the tool. The researchers suggested a short form of the tool may increase user completion and uptake. Thus in 2018, a team of researchers at the University of Florida, constructed and validated a short form of the Fitness-to-Drive Screening Measure. The 21-item Fitness-to-Drive Screening Measure Short-Form has been shown to be a valid and reliable screening tool for identifying at-risk older drivers. For more information on the validity and reliability of the short form, please refer to the paper Construction and validation of the 21-item fitness-to-Drive Screening Measure Short Form.
